The estimated salary for this role is $85,000 - $110,000 per year, based on national averages and industry standards. This figure may vary depending on factors such as location, experience, and qualifications.
Job Description
As a Home Health Case Manager, your primary responsibility will be to perform visits and coordinate client care, serving as the point of contact for all disciplines involved in providing care to patients. You will also oversee the frequency of visits for each episode and consult with physicians and office staff as needed. Additional responsibilities include accurate OASIS collection, ensuring medication profiles remain current, reporting lab values to physicians in a timely manner, attending weekly case conferences, and participating in monthly case manager meetings.
Required Skills and Qualifications
To succeed in this role, you must meet the following requirements:
- Be currently licensed as a Registered Nurse (RN) in the state of employment.
- Have a minimum of one year of clinical experience.
- Demonstrate knowledge and skill in current nursing practice.
- Possess a valid state driver's license and automobile liability insurance.
