About the Role

We are seeking a highly skilled Physical Therapy Assistant to join our team at Reliant Rehabilitation. As a Physical Therapy Assistant, you will work under the direction of a licensed Physical Therapist to provide high-quality physical therapy services to patients in skilled nursing facilities.

Responsibilities
  • Provide physical therapy services to patients, including ongoing treatment and case management responsibilities.
  • Work collaboratively with licensed Physical Therapists to develop and implement individualized treatment plans.
  • Maintain accurate and detailed records of patient progress and treatment plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an APTA-accredited school of physical therapy.
  • Valid Physical Therapy Assistant state license.
  • Computer literacy and proficiency in clinical documentation.
  • Basic internet navigation skills.
