**Our Team:**
We are seeking a dedicated RN Hospice Care Specialist to join our team of experienced professionals. As a member of our team, you will work collaboratively with interdisciplinary teams to develop and implement individualized plans of care. Your expertise will be valued in creating a palliative milieu that promotes a smooth transition at end-of-life.
**Key Responsibilities:**
* Assess patient needs and develop plans of care that prioritize comfort and reduce pain and suffering.
* Educate patients, families, and caregivers on end-of-life care and self-care techniques.
* Collaborate with healthcare teams to ensure seamless transitions and optimal outcomes.
