About the Role

As a Registered Nurse Home Health at Elara Caring, you will play a vital role in delivering high-quality care to patients in their preferred home setting. Our team is dedicated to providing compassionate and dedicated healthcare services, and we are seeking a skilled and experienced nurse to join our mission.

Key Responsibilities
  • Assess and evaluate patient needs, developing personalized plans of care to address physical, psychosocial, and environmental factors.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and families.
  • Conduct regular home visits to monitor patient progress, identify potential issues, and make adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal outcomes.
Requirements
  • Current, unrestricted state RN license.
  • 1+ year experience in a clinical care setting as a nurse.
  • Able and willing to travel within branch/office coverage area.
  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
  • Must be able and willing to travel 50%.
  • Associates Degree or Bachelor's Degree in Nursing is preferred.
  • Previous Home Health or Hospice Experience is preferred.
  • Previous experience with HomeCare HomeBase is preferred.
