Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at the Aleda E Lutz Department of Veterans Affairs Medical Center. As a Clinical Pharmacist, you will play a critical role in providing optimal drug therapy to our patients and ensuring their safety and well-being.

Responsibilities
  • Provide appropriate selection of drug therapy based on pharmaceutical principles of pharmacokinetics and pharmacodynamics
  • Monitor for efficacy, side effects, and clinical outcome, and advise prescribers as necessary
  • Provide patient-specific therapeutic drug monitoring and communicate relevant findings and/or recommendations to other healthcare providers
  • Perform continuous evaluation of prescribed medications to ensure optimal drug therapy
  • Provide medication counseling to patients and complete medication histories, including patient interviews
  • Serve as a drug information resource by providing up-to-date drug information to prescribers, other healthcare professionals, patients, and caregivers
  • Participate in patient care rounds, review charts, evaluate pertinent laboratory data, drug-drug and drug-nutrient interactions, monitor for adverse drug effects, and screen for allergies
  • May perform medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications
  • Report adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS reporting program
  • Participate in the medical center medication utilization evaluation (MUE) program
  • Recommend formulary alternatives and complete non-formulary consults as assigned
  • Participate in emergency preparedness activities
  • Provide pharmacokinetic consultation, including dosing vancomycin and aminoglycosides, in accordance with local policy
  • Implement P&T Committee policies, including therapeutic interchanges and automatic substitutions, and provide patient and provider education
  • Manage recalls and medication shortage situations by substituting alternate dosage strengths and instructions of the same medication to equal the prescribed dose and schedule
  • Enter supply orders appropriate for patient care per facility policy
  • Manage various actions, as appropriate, for medication orders, including flagging prescriptions, managing view alerts, using the "hold medication" function, and generating unsigned discontinued orders using Service Reject
  • Extend refills until next scheduled appointment under the provider's name per facility policy
  • Serve as a role model and preceptor to pharmacy students and residents, volunteers, technicians, and students
  • Assume an active role in staff development of peers and provide educational in-services to nursing, medical staff, or other ancillary services
Requirements
  • Must be a U.S. Citizen
  • Must be a licensed pharmacist in a State, Territory, Commonwealth of the United States, or the District of Columbia
  • Must be proficient in written and spoken English
  • Must pass a background investigation and obtain a security clearance
  • Must be able to work in a fast-paced environment and make sound judgments in a timely manner
  • Must be able to communicate effectively with patients, healthcare providers, and other stakeholders
Preferred Qualifications
  • Experience working in a clinical pharmacy setting
  • Experience with medication therapy management and patient care rounds
  • Experience with medication reconciliation and adverse drug event reporting
  • Experience with pharmacokinetic consultation and medication management
  • Experience with staff development and educational in-services
