The Cardiac Rehabilitation Registered Nurse plays a crucial role in overseeing and managing the clinical aspects of the rehabilitation program. This position is essential for ensuring participant safety and effective communication with the Medical Director and other healthcare professionals.
Key Responsibilities:- Facilitates the screening and intake process for potential participants.
- Determines participant eligibility with guidance from the Medical Director.
- Maintains accurate electronic health records and collects necessary data throughout the program duration.
- Conducts comprehensive health assessments at baseline and prior to each intervention session.
- Collaborates with specialists and resources during the intake process.
- Handles reimbursement requests as per payer guidelines.
- Achieves super user status in the electronic health record system and provides support to the team.
- Coordinates the development and evaluation of individualized treatment plans.
- Conducts ongoing health assessments and updates treatment plans as necessary.
- Leads weekly interdisciplinary team meetings and care conferences.
- Degree in Nursing (Diploma, Associate, Bachelor, Masters, or PhD).
- Active Registered Nurse license in good standing.
- Minimum of two years of experience in adult cardiovascular nursing or cardiac rehabilitation.
- Preferred formal education in cardiac rehabilitation or exercise physiology.
- Current certification in Advanced Cardiac Life Support (ACLS).
- Proficient in computer skills.
- Strong organizational and time management abilities.
- Excellent interpersonal communication skills with a compassionate approach.
- Public speaking proficiency and knowledge of adult education principles.
- Willingness to collaborate effectively within a multidisciplinary team.
