We are currently seeking a dedicated individual to take on the role of Registered Nurse. In this position, you will oversee resident care, conduct care conferences, assist with resident evaluations, and ensure that residents receive appropriate levels of care. You will also be responsible for monitoring medication administration and care plans, auditing medication carts, and maintaining resident files. This role requires a blend of administrative skills and leadership capabilities to guarantee that our residents receive the highest quality of care.
Key Responsibilities:
- Evaluate residents prior to their admission when possible.
- Conduct visits and reassessments for current residents in acute care or other long-term care environments.
- Implement the nursing process through thorough assessment, planning, execution, and evaluation of resident care, as demonstrated by staff observations.
- Oversee medication management by collaborating with pharmacy and team members to ensure accurate medication orders and availability.
- Maintain communication with resident Power of Attorneys and Primary Care Physicians regarding any medication modifications.
- Monitor electronic documentation, including observations and incident reports, and follow up with relevant directors on any incomplete reports or significant clinical concerns.
- Conduct monthly reviews of PRN psychotropic medications and quarterly assessments of routine psychotropic medications.
- Engage in auditing processes as necessary and participate in regulatory compliance activities as directed.
Qualifications:
- Bachelor's or associate degree in nursing is preferred.
- Must possess a current, unencumbered active license to practice as an RN or LPN in the state of Wisconsin or be qualified to obtain such a license prior to employment.
- Preferred experience includes at least one year in Long-Term Care settings.
