Job Summary

My Ace Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide high-quality patient care in a home environment. As a key member of our team, you will be responsible for delivering exceptional care to our patients and their families.

Key Responsibilities
  • One-on-One Patient Care: Provide individualized care to patients in their homes, ensuring their physical and emotional needs are met.
  • Assessment and Documentation: Conduct thorough assessments of patients, documenting their vital signs, medication, and allergies accurately and efficiently.
  • Medication Management: Verify medication milligrams, last day dosed, take-outs received, phase level, and guest doses to ensure patient safety.
  • Health Promotion and Education: Teach family members about patient care, medication management, and healthy habits to promote overall well-being.
  • Communication and Interpersonal Skills: Demonstrate excellent communication and interpersonal skills to build strong relationships with patients, families, and healthcare team members.
Requirements
  • Education and Licensure: High school diploma or equivalent and a valid LPN or RN license in the state of practice.
  • Experience: At least 12 months of hands-on nursing experience, with a focus on home healthcare and patient care.
  • Certifications: Current CPR certification and proficiency in the use of electronic medical records (EMRs) and computers.
  • Physical and Emotional Demands: Ability to work in a fast-paced environment, lift patients, and provide emotional support to patients and families.
What We Offer
