Job Summary

We are seeking a skilled Registered Nurse or Licensed Practical Nurse to join our team. As a key member of our care team, you will be responsible for providing exceptional care to our clients, while working closely with our staff to ensure seamless delivery of services.

Key Responsibilities:
  • Monitor and supervise the care of clients with complex health needs
  • Review referrals from care coordinators, evaluate client needs, and assign appropriate staff to provide care
  • Develop and revise client care plans as needed, and communicate revisions to staff
  • Supervise personal support aides and licensed practical nurses
  • Complete clinical records, including progress notes and supervisory visit entries
  • Review and co-sign documentation of LPN supervisory visits
  • Communicate client care needs with other providers, caregivers, and care coordinators
  • Make recommendations to care coordinators regarding client services
  • Conduct face-to-face visits with clients to re-evaluate their needs
  • Arrange for orientation and in-service training of staff
  • Educate clients, caregivers, and care coordinators about client health status and needs
Requirements:
  • CPR/First Aid certification
  • Certificate showing Negative for TB
  • Valid driver's license
  • Photo copy of Security Card
  • Current proof of auto insurance
  • Reliable transportation
  • Certification/license in the practice of Nursing (RN, LPN, or CNA)
  • Possible Federal fingerprinting requirement
Preferred Qualifications:
  • Minimum of two years' experience in home health services or a related field
  • Ability to supervise personal support aides
  • Ability to communicate effectively
  • Working knowledge of the Rules and Regulations for Private Home Care Providers
  • Working knowledge of the policies and procedures of the CCSP, SOURCE, ICWP, and the GAPP Medicaid Waiver Program
