POSITION OVERVIEW:
The Social Work Case Manager plays a vital role in collaborating with the healthcare team to ensure seamless care for patients. This position focuses on minimizing delays and preventing service denials while providing essential support to inpatients.
KEY RESPONSIBILITIES:
- Coordinate with community organizations to address issues such as abuse, neglect, substance use, and mental health challenges.
- Facilitate connections to financial resources and support services for patients and their families.
- Lead initiatives to enhance patient care through effective communication and collaboration with healthcare professionals.
QUALIFICATIONS:
- A Master’s degree in Social Work is mandatory.
- Experience in Social Work ranging from 2 to 4 years is essential.
- Prior experience in Medical Social Work is highly desirable.
- Certification as a Licensed Clinical Social Worker (LCSW-C) is required.
