About the Role: We are looking for an experienced Healthcare Navigator Lead to join our team at Community Health Network. As a key member of our multi-disciplinary team, you will play a vital role in ensuring that patients receive timely identification of their needs and coordination of patient services. Your primary responsibility will be to educate patients on disease processes, goal setting, and monitoring patients identified for wellness, transitions care, chronic care, and high-risk care management programs.

The estimated salary for this position is $90,000 - $105,000 per year, depending on experience. We require two years or more of Oncology nursing experience, with OCN certification preferred but not required upon hiring. *OCN or ONN-CG certification required within timeframe established upon hire.* You should have three years of nursing experience, with five years being preferred. Electronic medical records (EMR) skills are a serious plus, as well as experience in case/care management roles, home health, or other independent roles requiring nursing assessment, critical thinking, and application.
