We are seeking a skilled Clinical Care Coordinator to join our team at Adventist Health. As a key member of our care management team, you will be responsible for coordinating referrals and pre-authorizations for outpatient services, ensuring seamless patient care experiences. This role requires strong organizational and communication skills to work effectively with physicians, insurance carriers, patients, and other healthcare professionals.

Key Responsibilities
- Coordinate referrals and pre-authorizations for outpatient services ordered by physicians
- Work closely with multidisciplinary teams to ensure effective patient care coordination
- Perform initial patient assessments, including physical, psychosocial, safety, and environmental assessments
- Ensure accurate and timely documentation of orders and changes in care

Requirements and Qualifications
- Bachelor's Degree in Nursing (BSN): Preferred
- Experience in a care management role: Preferred
- Registered Nurse (RN) licensure in the state of practice: Required
- Basic Life Support (BLS) or HeartCode BLS (HBLS) or Healthstream Basic Life Support (HS-BLS): Required

Compensation and Benefits
The estimated annual salary for this position is around $80,000-$105,000, based on experience and qualifications. As an Adventist Health associate, you can expect a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
