We are seeking a skilled Oncology Case Manager to join our team at Kaiser Permanente in Portland, Oregon.

About the Job

This is a full-time position requiring a Registered Nurse with a minimum of 5 years of experience in acute care or ambulatory care/clinic/extended setting, including 2 years in case management/care coordination role. The ideal candidate will have strong clinical nursing and leadership skills, as well as excellent communication and problem-solving abilities.

Responsibilities
  • Utilize regional population stratification information and processes to identify appropriate members for enrollment into case management in collaboration with clinicians and healthcare team.
  • Independently and proactively complete chart reviews, screening calls and full assessments related to the anticipated level of care and document findings using standardized approved documentation tools.
  • Triage findings from member assessments, identifying needs and issues, engage patients to define a plan of care and appropriate level of self-management and interventions.
Requirements
  • A valid Registered Nurse License (Washington) within 6 months of hire or Compact License: Registered Nurse within 6 months of hire.
  • A valid Registered Nurse License (Oregon) within 6 months of hire.
  • Ability to work independently in an unstructured environment with minimal supervision.
  • Able to type 20 words per minute.
Estimated Salary Range:$105,000 - $145,000 per year
