Overview: The Clinical Nurse Specialist for Advanced Lung Disease (ALD) acts as the clinical liaison within our ALD Program in an outpatient environment. Under the guidance of ALD healthcare providers, the specialty nurse will oversee clinical operations in alignment with established orders, policies, and protocols.
Key Responsibilities:
- Manage incoming triage communications, including clinical evaluations and the management of symptoms and treatment side effects.
- Conduct nursing assessments to gather data and clinical insights to support patient care.
- Coordinate sick visits with healthcare providers as necessary and ensure accurate documentation in the medical records.
- Deliver empathetic and focused nursing support to patients, their caregivers, and family members, while providing educational resources.
- Utilize clinical judgment grounded in established evidence-based policies, procedures, and guidelines for informed decision-making.
Assist with onboarding new patients by compiling necessary documentation and diagnostic test results prior to appointments. Facilitate referrals to external services such as hospice, palliative care, and lung transplant.
Engage in planning and coordination for various pulmonary support groups, including those for pulmonary hypertension and pulmonary fibrosis. Attend meetings with group participants.
Coordinate lab testing scheduling and medication adjustments for patients on high-risk medications, adhering to established protocols. Report any abnormal findings to ALD providers and ensure patient follow-up and documentation.
Assist with enrollment in specialty pharmacy-provided medications, manage prior authorizations, insurance appeals, and financial assistance. Provide prescription refills as required.
Represent Piedmont Healthcare to national nonprofit organizations and accrediting bodies, such as the Pulmonary Fibrosis Foundation and the Pulmonary Hypertension Association. Participate in relevant committees and attend national conferences.
Qualifications:
Minimum Education Required:
Successful completion of an accredited registered nursing program or a bachelor's degree in nursing.
Minimum Experience Required:
No prior experience necessary.
Minimum Licensure/Certification Required by Law:
State of Georgia Licensure as a registered nurse is mandatory.
