Position Overview


A dedicated professional responsible for directing, supervising, assessing, and delivering physical therapy services to patients in their residences, adhering to physician directives.

Key Responsibilities

1. Conduct thorough evaluations of patients utilizing diagnostic assessments of muscles, nerves, joints, and functional capabilities.


2. Develop a comprehensive assessment of the patient's condition during the initial evaluation and perform regular re-assessments, creating a detailed problem list and care plan that outlines treatment types and frequencies.


3. Recommend adjustments to the care plan to the physician as necessary.


4. Maintain accurate clinical documentation on the day of service in the medical records, ensuring all records and summaries are current.


5. Provide treatment aimed at addressing impairments and enhancing functional limitations, striving for maximum independence for the patient.


6. Educate and advise patients and their caregivers on the proper use of prosthetic, orthotic, and durable medical equipment (such as canes, walkers, and wheelchairs).


7. Engage patients and caregivers in discussions regarding the treatment plan and objectives.

8. Attend physical therapy staff meetings and participate in in-service training sessions.

9. Contribute to case conferences and assist in the ongoing development and periodic updates of the physician's Plan of Treatment.

10. Ensure adherence to safety protocols and proper maintenance of equipment.

11. Perform additional duties as assigned.

Educational Qualifications

Graduate of an accredited Physical Therapy program.

Qualifications and Requirements

Possession of a current, valid Physical Therapy license issued by the appropriate state authority.

A minimum of one year of practical experience as a licensed physical therapist.

Work Environment

The Physical Therapist primarily operates in a non-office environment and may encounter outdoor conditions.

Physical Demands

The role is classified as medium work, which entails:


Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects, including patients.


Medium work involves both sitting and standing.

Daily Physical Requirements

While performing the duties of this position, the employee is regularly expected to:
