As a key member of our team, you will be responsible for ensuring the delivery of high-quality care to our patients while maintaining compliance with contractual and regulatory requirements.
Key Responsibilities- Oversee the implementation of Care Management Services, ensuring alignment with organizational goals and objectives.
- Collaborate with cross-functional teams, including clinical staff, program managers, and leadership, to ensure seamless delivery of care.
- Develop and maintain relationships with community partners, healthcare providers, and other stakeholders to ensure access to care for our patients.
- Monitor and analyze program data to identify trends, opportunities for improvement, and areas for growth.
- Develop and implement strategies to improve patient outcomes, satisfaction, and engagement.
- Ensure compliance with contractual, federal, and state regulatory requirements, including the Ryan White Care Act.
- Provide clinical guidance and operational support to care management teams, including training and coaching on best practices.
- Develop and maintain policies, procedures, and guidelines to ensure consistency and quality in care management services.
- Participate in quality improvement initiatives, including data analysis, process improvement, and quality assurance activities.
- Bachelor's or Master's degree in Nursing (BSN)
- Minimum of 3 years of case management experience, preferably in a healthcare setting
- 2+ years of management experience, preferably in a healthcare setting
- Ability to travel up to 50% of the time, as needed
- Strong clinical knowledge and expertise in care management, including the Ryan White Care Act
- Excellent communication, interpersonal, and leadership skills
- Ability to work in a fast-paced environment, prioritize tasks, and manage multiple projects simultaneously
