Job Summary

We are seeking a skilled Nurse Practitioner to join our team in West Portsmouth, Ohio. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work in a variety of settings, including home visits and telehealth. You will be responsible for conducting comprehensive health assessments, developing care plans, and providing education and support to patients and their families.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
  • Develop and implement care plans to improve patient outcomes
  • Provide education and support to patients and their families
  • Collaborate with primary care physicians and other healthcare professionals to ensure seamless care
  • Work in a variety of settings, including home visits and telehealth
Requirements
  • Masters degree in nursing or related field
  • Current RN and NP licensure in state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
