Job Summary

We are seeking a skilled and compassionate LPN to join our team at Liberty Park. As a nursing professional, you will play a vital role in enriching the lives of our residents and helping them live their best life.

Key Responsibilities
  • Medication Management: Ensure accurate and timely administration of medications to our residents.
  • Fall Prevention: Implement strategies to prevent falls and ensure resident safety.
  • Skin Breakdown Prevention: Monitor and prevent skin breakdown in our residents.
  • Incident Reporting: Accurately document and report incidents to ensure regulatory compliance.
  • State Regulatory Compliance: Maintain knowledge of and adhere to state regulations and guidelines.
Resident Health and Wellness
  • Complete State-Specific Assessments: Conduct regular assessments to identify resident needs and develop care plans.
  • Implement Inspirit Senior Living Philosophy: Ensure that our philosophy and principles are implemented in all aspects of care.
  • Monitor 24-Hour Reports: Review reports for physical and behavioral changes and ensure proper documentation.
  • Communicate with Families and Physicians: Maintain open communication with families and physicians to ensure resident needs are met.
  • Wellness Visits: Conduct regular wellness visits to identify resident needs and provide education.
  • Illness Visits: Provide care and support to residents with illnesses or injuries.
  • Liaison with Health Services: Facilitate communication and coordination with health services, such as home health and mental health associations.
  • Liaison with Pharmacy: Ensure effective services and regulatory compliance with the pharmacy.
  • Coordinate Services: Assist with coordination of services with other healthcare facilities to ensure continuum of care.
  • Emergency First Aid: Provide emergency first aid to residents and team members as needed.
  • Infection Control: Monitor and prevent infections in our residents.
  • Consultation and Technical Assistance: Provide consultation and technical assistance to team members as needed.
  • Resident Confidentiality: Maintain resident confidentiality and coach others on its importance.
Requirements
  • Education: LPN License
  • Knowledge/Skills/Abilities: Good organizational and time management skills, compassionate attitude, good judgment, problem-solving, and decision-making skills, and ability to effectively read, write, and communicate in the language of the residents.
