Job Summary:

As a Licensed Practical Nurse (LPN) at LifeBridge Health, you will play a vital role in providing high-quality patient care in our long-term care setting. Under the supervision of the Registered Nurse/Director of Nursing, you will utilize your nursing skills and knowledge to develop and implement individualized care plans that meet the unique needs of our residents.

Key Responsibilities:

  • Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
  • Develop and implement individualized care plans that address the physical, emotional, and social needs of our residents.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Requirements:

  • Current LPN licensure in the state of Maryland.
  • Basic Life Support (BLS) certification.
  • 1+ year of experience in long-term care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
