Job Summary:
We are seeking a highly skilled and experienced Health Homes Supervisor to join our team at South King Healthcare Services, SPC. The successful candidate will be responsible for providing coordinated care to patients by developing, monitoring, and evaluating interdisciplinary care plans.
Key Responsibilities:
- Coordinate and manage all aspects of patient care within the home
- Implement and oversee discharge planning procedures
- Run Scripts Audit for other staff members in the Health homes department
- Conduct diagnostic evaluations and provide necessary care to residents
- Collaborate with medical professionals, including ER staff, to ensure comprehensive care
- Maintain accurate medical records and documentation
- Provide inhalation therapy and other medical interventions as required
Requirements:
- Previous experience in a clinic or nursing setting is preferred
- Proficiency in medical records management and discharge planning
- Strong understanding of health homes DSHS program and supervisory duties
- Ability to conduct diagnostic evaluations and provide appropriate care coordination choices for All WA-State Medicaid and Medicare Providers
