Job Summary

As a Transplant Coordinator with Henry Ford Health System, you will play a vital role in the care of transplant recipients and donors. You will participate in medical, administrative, research, and teaching duties as an active member of the multi-disciplinary transplant team.

Key Responsibilities
  • Develop and implement IPD critical pathways
  • Coordinate IPD discharge planning
  • Facilitate and manage the use of home care
  • Triage OPD and IPD medical issues with referral to appropriate healthcare professionals
  • Communicate with insurance companies and referring physicians
  • Participate in quality assurance projects
  • Promote health maintenance education and follow-up
  • Manage donor and recipient family and patient interactions
Requirements
  • Bachelor's degree in Nursing preferred
  • Minimum of two years clinical nursing experience in an inpatient or outpatient setting
  • Organizational and interpersonal skills to effectively communicate with patient care personnel and relate to patients and their families
  • Analytical ability and motivation necessary to effectively plan, deliver, and implement plans of care in acute and outpatient situations
Certifications/Licenses
  • Current RN license in the State of Michigan
