Job Summary

We are seeking a Community Health Specialist to work with our team in Oahu, HI. As a Field Based Community Health Worker, you will act as a liaison with Medicaid members to ensure they receive the best possible care. Your responsibilities will include providing home and social assessments, member education, and case management planning.

Responsibilities:
  • Provide direct support to visitors at the Community Connection Center to assist with service navigation and referrals
  • Maintain a clean and safe environment at the center
  • Participate in community coalitions and workgroups to address health disparities and enhance UHC services
  • Assess, plan, and implement care strategies for patients
  • Utilize company and community-based resources to establish effective case management plans
Requirements:

To be successful in this role, you must have a Bachelor's Degree in Social Work or a related field, or equivalent experience. You should also have 1+ years of experience working in a community health setting and knowledge of local resources, culture, and values. Intermediate computer skills, including Microsoft Outlook and Teams, are required.

Preferred Qualifications:
  • Community Health Worker Accreditation
  • Experience working in Managed Care
  • Knowledge of Medicaid/Medicare population
