About the Role:

We are seeking an experienced LPN Medication Care Manager to join our team in Wayne NJ. As a Medication Care Specialist, you will play a vital role in providing high-quality care and services by administering medication and treatments in a safe and organized manner.

Job Responsibilities:

  • Administer medications and treatments according to established protocols and procedures
  • Review and update resident medication records to ensure accuracy and compliance
  • Collaborate with the Resident Care Director and Wellness Nurse to receive medication updates and coordinate care plans
  • Conduct regular audits to ensure medication carts and storage areas are secure and organized
  • Participate in daily Cross Over meetings to discuss resident care and address any concerns
  • Develop and maintain Individualized Service Plans (ISPs) and monthly updates to promote resident autonomy and well-being
  • Document and report all incidents, accidents, and safety concerns to ensure a safe environment for residents and staff

Required Skills and Qualifications:

  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) credential
  • CPR and First Aid certification
  • Previous experience working with seniors and in a healthcare setting
  • Ability to make sound judgments and decisions in a fast-paced environment
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Estimated Salary: $65,000 - $80,000 per year
