Amergis is seeking a highly skilled Licensed Clinical Social Worker to join our team. As a key member of our multi-disciplinary team, you will provide critical social work intervention and counseling services to patients and clients in a variety of settings.
Key Responsibilities- Provide necessary social work intervention to patients and clients, including counseling and crisis intervention.
- Assess patient psychosocial needs, plan for, evaluate, and modify interventions to meet established patient/client goals.
- Collaborate with all disciplines to plan and evaluate team goals for each patient/client.
- Facilitate patient/client and family referrals to community agencies when appropriate.
- Provide financial counseling to achieve treatment goals established by the multi-disciplinary team.
- Respond to crisis situations, including domestic violence and child abuse.
- Provide witness services and evidence in court cases.
- Visit schools to assess academic concerns.
- Work with government agencies to help clients apply for Medicare and Medicaid when appropriate.
- Arrange home visits to evaluate patient/client health, home environment, and family interaction/support.
- Assist patient/family/caregivers in planning for present and future psychosocial needs.
- Observe and report patient progress and/or changes in patient status to physician and appropriate multi-disciplinary team members.
- Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education.
- Active license as an LCSW in the state of practice.
- Minimum of one year of social work experience.
- Complies with all relevant professional standards of practice.
- Participation and completion of Amergis' Competency program when applicable.
- Current CPR if applicable.
- TB questionnaire, PPD, or chest x-ray if applicable.
- Current Health certificate (per contract or state regulation).
- Must meet all federal, state, and local requirements.
- Successful completion of new hire training as applicable to job site.
- Understand patient confidentiality and HIPAA requirements.
- Ability to effectively elicit/provide information to and from appropriate individuals via strong communication skills; proficiency in the English language is required.
- Computer proficiency required.
- Must be at least 18 years of age.
