Key Responsibilities- Screen patient charts to ensure medication orders are aligned with the chart note as well as lab work, vitals, allergies, etc.
- Process new medication orders in a timely and accurate manner.
- Fill prescription orders when needed, ensuring accuracy and attention to detail.
- Check filled prescription orders for accuracy and completeness.
- Educate patients on medication therapy and provide clear instructions.
- Provide patient education on medication use, side effects, and potential interactions.
- Work collaboratively with the pharmacy team and providers to ensure timely and accurate pharmaceutical services.
- Answer provider questions regarding drug therapy and provide guidance when necessary.
- Recognize life-threatening emergencies and initiate resuscitation measures as appropriate.
- Recognize adverse reactions to medications, treatments, and procedures, and initiate interventions as needed.
- Answer and screen telephone calls, take refill requests, and input outside prescriptions into the system.
- Participate in quality improvement initiatives, including the Functional Improvement, Pain Reduction Committee and the Pharmacy and Therapeutics Committee.
- Perform controlled substance audits as directed.
- Assist with inventory management and help put away orders as needed.
- Conduct pill counts and communicate with patients regarding their medication use.
To be successful in this role, you will need to have a strong foundation in pharmacy practice, excellent communication skills, and the ability to work effectively in a team environment.
