Position: Licensed Practical Nurse
The role of the Licensed Practical Nurse (LPN) involves a significant responsibility in delivering nursing care and support to residents. This position operates under the direct supervision of a Charge Nurse and may oversee the activities of Nursing Assistants and other support staff as necessary.
Key Responsibilities:
- Assist new residents in acclimating to the unit and understanding the philosophy and objectives of Willow Point Rehabilitation and Nursing Home.
- Report any incidents or changes in residents' conditions to the Charge Nurse and document these observations in the residents' progress notes.
- Engage in discussions regarding clinical matters related to resident care with the Charge Nurse.
- Collaborate as part of an interdisciplinary team to aid in the development, execution, and ongoing assessment of individualized care plans.
- Monitor, document, and communicate any changes in residents' health status.
- Update care plans as necessary to reflect the current needs of residents.
- Implement therapeutic treatments as prescribed.
- Administer and manage medications (excluding IV medications) while monitoring for any side effects and overall effectiveness.
- Ensure proper handling and accountability for controlled substances.
- Maintain accurate nursing care and administrative records.
- Participate in staff meetings and continuing education programs.
Essential Knowledge, Skills, and Abilities:
- Proficient understanding of contemporary nursing practices.
- Competence in applying practical nursing techniques and procedures for resident care.
- Effective communication skills, both verbal and written.
- Ability to foster and maintain collaborative working relationships.
- Dependability, resourcefulness, keen observational skills, and professionalism.
- Capability to operate medical equipment, such as suction machines and oxygen units.
- Ability to maintain detailed records and generate reports.
- Physical fitness suitable for the demands of the role.
Minimum Qualifications:
- Completion of a nursing program recognized by the New York State Education Department, qualifying for Licensed Practical Nurse licensure in New York State; or
- Completion of a nursing program recognized by the New York State Education Department and holding a limited permit to practice as a Licensed Practical Nurse in New York State, with licensure to be obtained within six months of appointment.
