RN Case Management Lead

We are seeking an experienced RN to lead our case management team and deliver high-quality patient care. As a key member of our team, you will be responsible for coordinating activities, prioritizing workflow, and promoting decision-making and problem-solving.
Key Responsibilities- Lead the delivery of quality patient care and adherence to policies, procedures, and case management protocols.
- Actively participate in the process of monitoring, guiding, and evaluating nursing staff performance.
- Serve as a first point of escalation for team members needing consultation for and/or management of patients requiring complex or advanced treatment/care.
- Provide onboarding support to new associates and functions as a preceptor to new associates, working with management to develop and adjust the orientation plan.
- Provide input into hiring, training, directing, development, and evaluation of staff as well as day-to-day operational support to management as needed.
- Licensure/Certification/Registration: Registered Nurse credentialed from the Indiana Board of Nursing required.
- Education: High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree required.
- Work Experience: 1 year of experience required.
- Licensure/Certification/Registration: Case Manager credentialed from the American Case Management Association (ACMA) or Commission for Case Manager Certification (CCMC).
- Education: Master's degree preferred.
