**Job Description:**

The RN Case Manager will be responsible for providing high-quality patient care, coordinating with interdisciplinary teams, and ensuring that patients receive optimal support during their end-of-life journey. The ideal candidate will possess strong communication skills, be able to work effectively in a team environment, and have a passion for delivering compassionate care.

**Key Responsibilities:**

  1. Assume primary responsibility for patient caseloads.
  2. Develop and implement individualized care plans.
  3. Monitor patient progress and adjust care plans as needed.
  4. Communicate effectively with patients, families, and healthcare teams.

**Requirements:**

  • Graduate from an accredited registered nursing program.
  • Licensed as a registered nurse in the state of practice.
  • Current CPR and First Aid Certification.
  • Knowledgeable of quality assessment and assurance procedures.
