Job Summary

The Hospice Clinical Liaison is a key member of our team, responsible for promoting the Compassus philosophy and values. This role requires a strong understanding of hospice care and the ability to effectively communicate with patients, families, and healthcare professionals.

Key Responsibilities
  • Attend and participate in staff meetings, educational programs, and community events to stay up-to-date on industry developments and best practices.
  • Communicate with referral sources to identify patients who may benefit from our services and provide education on hospice care.
  • Assess patient needs and disease status, conferring with attending physicians and hospice physicians to establish eligibility for care.
  • Facilitate patient discharge from hospital to home, coordinating transportation, DME, medication needs, supplies, and admission by a home hospice nurse.
  • Oversee the GIP patient admission process, working closely with facilities and hospitals to identify, transition, and admit patients appropriately for general inpatient services.
Requirements
  • RN License required, Bachelor's degree in Nursing highly preferred.
  • Two to three years of related experience required, with hospital and/or long-term care clinical experience highly preferred.
  • Experience in healthcare marketing preferred.
Skills and Abilities
  • Strong written and verbal communication skills, with the ability to effectively present information to leaders, team members, investors, and external parties.
  • Ability to understand, read, write, and speak English, with a strong understanding of hospice philosophy and values.
Work Environment

The demands of this role necessitate a team member who can effectively perform essential functions, with adaptations made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.

Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
