Physical Therapist Assistant

We are seeking a skilled Physical Therapist Assistant to join our Homecare team in Monmouth County. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our team.
Responsibilities- Implement individualized treatment plans for patients in their homes, under the supervision of a licensed Physical Therapist.
- Contribute to the development of treatment programs based on patient observations and caregiver input.
- Provide ongoing treatment focused on achieving short and long-term goals established by the Physical Therapist.
- Monitor treatment effectiveness and adjust plans as needed.
- Develop a rapport with patients and caregivers to foster a positive environment for goal achievement.
- Implement treatment plans using correct clinical skills and techniques while maintaining patient and caregiver safety.
- Assure accurate documentation of patient progress and communication with the managing Physical Therapist.
- Provide comprehensive education to patients and caregivers based on their identified needs.
- Communicate effectively with patients and caregivers in a clear and concise manner.
- Act as a liaison between healthcare team members to ensure effective coordination and communication.
- Document case communication with other healthcare team members as needed.
- Participate in discharge planning and address problem areas with patients and the managing Physical Therapist.
- Comply with the scope of practice for Physical Therapist Assistants, including review of PT evaluations and re-assessment of patients every six visits or fourteen days.
- Respond to voice mail and secure messages in a timely manner.
- Meet productivity standards and communicate effectively with patients to ensure timely scheduled visits.
- Participate in team meetings, in-services, and training programs.
- Have knowledge of and ability to use and assist patients with varying DME, including wheelchairs, mechanical lifts, slings, splints, prosthesis, and orthoses.
- Completion of a Two-Year Physical Therapist Assistant Program from a CAPTE Accredited Two-Year Associate Degree Program.
- One (1) Year Experience.
- Proficient in Microsoft Office Suite.
- Licensed Physical Therapy Assistant.
- AHA Basic Health Care Life Support HCP Certification.
