Job Title: Registered Nurse RN Home Health

Job Summary:

Banner Health is seeking a skilled Registered Nurse RN to join our Home Health team. As a Home Health RN, you will provide exceptional care to patients in their homes, focusing on medical intervention and creating one-on-one relationships with patients and their caregivers.

Responsibilities:

  • Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
  • Develop and implement individualized plans of care, including discharge plans, in collaboration with the patient, family, and healthcare team.
  • Provide direct patient care, coordinate care, delegate tasks, and supervise the activities of the healthcare team.
  • Evaluate patient progress, adjust plans of care as needed, and document assessment, planning, implementation, and evaluation in the patient record.
  • Collaborate with other healthcare professionals to ensure comprehensive care and promote continuity of care.

Requirements:

  • Current, valid RN license in the state of practice.
  • BLS certification.
  • Minimum one year of experience in an acute care hospital setting or home health care.

Preferred Qualifications:

  • Bachelor's degree in nursing.
  • Professional certification in home health care or a related field.
