Banner Health is seeking a skilled Home Health Registered Nurse to join our team. As a Home Health RN, you will provide exceptional care to patients in the comfort of their own homes.

About the Role

This is a part-time position with a minimum weekly commitment of 3/8 hour shifts (24 hours per week). Flexible schedule options are available, including 10-hour shifts and weekend shifts with a $3/hour differential.

Responsibilities
  • Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
  • Develop and implement individualized plans of care in collaboration with the multidisciplinary team.
  • Provide direct patient care, coordinate care, delegate tasks, and supervise the activities of the healthcare team.
  • Evaluate patient progress towards goals and expected outcomes in collaboration with other healthcare team members.
  • Document assessment, planning, implementation, and evaluation in the patient record.
Requirements

Must possess a current, valid RN license in the state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
