About the Role

We are seeking a skilled Certified Nursing Assistant to join our team at Providence PACE/ElderPlace in Irvington Village. As a CNA, you will play a vital role in providing high-quality care to our participants, fostering independence, freedom of choice, and dignity.

Key Responsibilities
  • Assist participants with activities of daily living, such as bathing, dressing, and grooming.
  • Provide personal care and support to participants, promoting their physical and emotional well-being.
  • Assist with mobility and transfers, ensuring the safety and comfort of participants.
  • Monitor and report any changes in participant condition, collaborating with healthcare professionals to ensure optimal care.
  • Participate in interdisciplinary team meetings to discuss participant care and develop individualized service plans.
Requirements
  • 3 months of experience in long-term care or related geriatric and/or rehabilitative care.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • Oregon Nursing Assistant Certification License upon hire.
Preferred Qualifications
  • 6 months of Assisted Living experience.
