Job Summary
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Home Health Nurse. As a Home Health Nurse, you will provide one-on-one patient care in a home environment, delivering specialized nursing care to medically complicated adult patients. Your expertise will be essential in assessing patients, coordinating care, and promoting health and wellness.
Responsibilities
- Provide one-on-one patient care in a home environment
- Deliver specialized nursing care to medically complicated adult patients
- Assess patients and coordinate care
- Health promotion, teaching, and training of patients and/or family members
- Must be able to work one weekend per month
Qualifications
- Must have a good standing license (LPN) in the state in which the clinician will practice
- Current CPR certification
- Must be willing to travel and visit adult patients in their home
