Key Responsibilities:

  • Provide outreach services, clinical assessments, and case management services to community members
  • Assist community members in identifying available resources, services, and treatment options
  • Link community members to services and provide ongoing assessment of their emotional well-being and clinical symptomology
  • Collaborate with the On P.O.I.N.T. team, law enforcement, and community service providers as needed
  • Participate in a multi-disciplinary treatment team for collaboration of client care
  • Provide direct assistance to consumers with obtaining basic needs
  • Provide crisis intervention and assessment as necessary
  • Co-respond in the community alongside an officer

Requirements:

  • Master's Degree in a Health and Human Services Field
  • LSW or LAC required
  • Experience in providing community outreach services
  • Knowledge of the Ocean County System of Care for adults and youth
  • Experience and understanding of Law Enforcement or experience working with law enforcement strongly preferred
  • Valid NJ Driver's License with less than six (6) points
  • Bilingual preferred
