Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Connecticut Institute for Communities, Inc. as a Patient Care Specialist in our Women's Health Department.
Key Responsibilities:
- Collect and document patient information to support healthcare providers in assessment and care planning.
- Administer medications and treatments as prescribed by healthcare providers, ensuring accurate and timely documentation.
- Monitor and record vital signs, medical history, and symptoms, providing critical information for patient care.
- Assist with physical exams and medical procedures as needed, adhering to licensure requirements.
- Provide education to patients and families on healthcare topics, promoting informed decision-making and self-care.
- Collaborate with healthcare professionals, including doctors, nurse practitioners, and other medical staff, to deliver high-quality patient care.
- Triage patient calls, using established protocols to determine appropriate care, including at-home management or medical visits.
- Develop and implement patient education plans, including after-hours care, triage, and referral services.
- Communicate with specialists to obtain letters and results, facilitating home therapies as determined by healthcare providers.
Requirements:
- High School Diploma or equivalent.
- Current LPN license in the state of Connecticut.
- Previous experience in pediatrics preferred.
- Strong communication and interpersonal skills, with bilingual abilities strongly encouraged.
- Ability to work in a fast-paced environment, with attention to detail and multitasking skills.
- Understanding of medical terminology and nursing protocols.
- Experience with electronic medical records, preferably eClinical Works.
