As a Clinical Nurse Care Coordinator, you will deliver primary healthcare services to adults within a community-focused, nurse-led health facility, catering to diverse and underserved populations. This role reports directly to the Clinical Director.

Key Responsibilities:
  • Administer high-quality nursing care and health education to patients across all age groups, adhering to established nursing protocols.
  • Serve as the care coordinator for patients managing chronic health conditions such as diabetes, hypertension, and hyperlipidemia.
  • Educate patients and their families about diagnoses and care plans.
  • Maintain effective communication with supervisors regarding significant patient issues.
  • Foster positive relationships with funding entities and community organizations.
  • Ensure clinical proficiency and competency, including age-specific knowledge, while demonstrating comprehensive understanding of procedures to provide quality primary healthcare.
  • Document nursing care and health services in accordance with organizational policies, ensuring accuracy and timeliness.
  • Coordinate patient care delivery among the healthcare team.
  • Collaborate with health center staff to optimize patient care and enhance patient flow and satisfaction.
  • Act as a resource for other staff members regarding patient care.
  • Engage as a team player, supporting colleagues with patient care and departmental tasks.
  • Maintain adequate inventory of medical supplies and medications, completing required monthly reports.
  • Assist in the preparation and documentation of medications as needed.
  • Administer medications accurately, following the five rights of medication administration and documenting as per health center policies.
  • Conduct maintenance checks on medical equipment and document findings.
  • Oversee and guide patient care activities, ensuring accountability.
  • Identify opportunities for performance improvement.
  • Stay informed on nursing and healthcare trends that influence practice.
  • Collaborate with the healthcare team to assess and prioritize patient and family needs, developing comprehensive care plans.
  • Participate in evaluating healthcare products, procedures, and standards.
  • Contribute to the assessment of health outcomes for patients within the health center.
  • Assist in maintaining a safe, secure, and orderly environment for patients and staff.
  • Adhere to OSHA standards and safety protocols.
Qualifications:
  • Ability to maintain thorough, accurate, and timely nursing documentation.
  • Knowledge of health promotion principles and disease prevention strategies.
  • Familiarity with medical terminology and relevant abbreviations.
  • Understanding of the Pennsylvania Nursing Practice Act.
  • Ability to effectively communicate medical information and present to small groups.
  • Experience in primary care, community health, or behavioral health is preferred.
  • Demonstrated capability in conducting comprehensive nursing assessments.
Education:
  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Current, unrestricted RN license from the Pennsylvania Board of Nursing.
  • Current CPR certification for Healthcare Professionals.
Exposure to communicable diseases, body fluids, and other conditions typical of a clinical environment is expected.
