Job SummaryWe are seeking a highly skilled RN Hospice Case Manager to join our team. In this role, you will be responsible for providing direct patient care services, evaluating patient needs, and developing personalized plans of care. Your compassion, empathy, and strong communication skills will enable you to build trusting relationships with patients and their families.
Responsibilities- Provide direct patient care services, including administering medications and performing wound care.
- Evaluate patient needs and develop personalized plans of care in collaboration with the interdisciplinary team.
- Act as a liaison between physicians, patients, and families, ensuring seamless communication and coordination of care.
- Collaborate with other healthcare professionals to provide comprehensive care and support.
- RN license in the state of employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, or a related field
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
