Job Summary

We are seeking an experienced Occupational Therapist to join our team as a Home-Based Care Specialist. As an Occupational Therapist with HarmonyCares, you will play a vital role in evaluating patient function and developing personalized treatment plans to improve independence and overall well-being.

Responsibilities
  • Evaluate patient function and develop treatment plans in collaboration with physicians and other healthcare professionals
  • Guide patients in prescribed therapeutic activities to improve independence and physical and mental functioning
  • Advise and instruct patients and their families on the use of assistive devices and adaptive equipment
  • Participate in performance improvement activities and discharge planning
  • Ensure delivery of Age-Friendly Care and the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility)
Requirements
  • Graduate of an occupational therapy curriculum accredited by the American Occupational Therapy Association
  • Current licensure as an Occupational Therapist in the state of practice with at least 2 years of experience
  • Active CPR certification
  • Maintain a valid driver's license, reliable vehicle, and good driving record
