Palliative Care Field Nurse Job Summary:
We are seeking a compassionate Palliative Care Field Nurse to join our team at Empath Health. As a Palliative Care Field Nurse, you will be responsible for providing high-quality patient care in various settings, including patients' homes, long-term care facilities, and hospitals.
Key Responsibilities:
- Conduct patient assessments to identify areas for improvement.
- Develop and implement personalized care plans to meet the unique needs of each patient.
- Manage pain and other symptoms using evidence-based practices.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Maintain accurate and up-to-date records of patient interactions and progress.
Requirements and Qualifications:
- Current, active RN licensure in Florida.
- Minimum one year of acute care experience, preferably in a hospital or home health setting.
- Strong understanding of palliative care principles and practices.
- Excellent communication and interpersonal skills.
