At Empath Health, we are seeking a compassionate and skilled RN - Triage/Field to provide high-quality patient care in Charlotte and DeSoto counties. The successful candidate will work closely with our team to deliver exceptional service and support to patients and their families.

About the Role

This is a full-time position working weekends from 11a-11:30p and one weekday night from 5p-1:30a, with occasional travel to Sarasota and Manatee counties. The ideal candidate will have a strong background in acute care nursing, preferably in a hospital or home health setting.

Key Responsibilities
  • Provide clinical, physical, educational, and emotional support to patients after hours, visiting homes, long-term care facilities, assisted living facilities, hospice houses, or hospitals as directed by the triage team.
  • Under the supervision of a licensed physician, conduct patient assessments, manage discomfort, and provide symptom relief using specialized nursing skills related to palliative and end-of-life care.
  • Utilize nursing interventions and teaching skills to maximize patient comfort and enhance the quality of life for patients and their families.
  • Attend patient deaths, offer bereavement support to caregivers and families, and perform post-mortem care as needed.
  • Document patient services accurately and timely within the electronic medical record, reflecting individualized plans of care and intervention outcomes.
Requirements
  • Current, unrestricted Florida licensure as an RN.
  • BSN or MSN degree preferred.
  • Minimum one year recent experience in acute care nursing, preferably in a hospital or home health setting; prior experience as a Hospice RN is desirable.
  • CHPN certification is strongly encouraged within three years of employment.
Compensation and Benefits
