Job Title: Medical Care Coordinator LPN

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Bradford Health Services, Inc. This is an excellent opportunity to make a meaningful difference in patients' lives while working in a dynamic and supportive environment.

About the Role:
As a Medical Care Coordinator LPN, you will play a critical role in coordinating and delivering quality medical care to patients. You will work closely with therapists and medical providers to ensure that patients receive the best possible care.

Responsibilities:
* Greet and direct patients who phone or visit the facility
* Schedule new and follow-up patient appointments
* Monitor and review patient schedules for next office appointments
* Provide appointment reminders and motivate patients to keep their appointments
* Coordinate physician and counselor schedules
* Obtain patient authorization for medical records release and HIPAA compliance
* Prepare patients for examination and treatment, including taking vital signs
* Interview patients and record medical intake information for providers
* Perform screening and medical procedures as ordered by physicians (urine drug screens, pregnancy tests, injections, etc.)
* Prepare exam and treatment rooms with necessary instruments and supplies
* Maintain cleanliness of waiting room and compile reports

Requirements:
* LPN required
* CPR certified or within 30 days of hire
* Clinical experience required: LPN
* Experience in Substance Abuse Treatment and/or Medication Assisted Treatment preferred

Compensation:
$55,000 - $65,000 per year based on experience and qualifications.
