Residential Care Coordinator

Our Residential Care Coordinator plays a crucial role in achieving this goal by providing supportive services and promoting stability, self-sufficiency, and independence.
Job Description:
The Residential Care Coordinator will increase rapport with clients through trauma-informed practices, creating a safe and welcoming environment that fosters stabilization, self-sufficiency, independence, and basic services to promote a healthier lifestyle.
Responsibilities:
- Answer phones and provide program information as necessary.
- Ensure the safety of clients and enforce facility rules through diligent monitoring and engagement with clients. Conduct bi-hourly head counts.
- Take vital signs. Monitor and document client prescription medications.
- Conduct urine collection for urinalysis, if needed.
- Monitor, educate, and assist clients in performing activities of daily living in accordance with goals and problem areas identified on individual treatment plans.
- Facilitate skills groups.
- Adhere to daily schedules.
- Attend staff meetings and training when scheduled.
- Utilize de-escalation skills to manage client crises by adhering to De-escalation Training.
- Complete all paperwork and reporting related to client and program activities accurately and in accordance with licensing and contract requirements, managed confidentially, and stored appropriately.
- Complete incident reporting immediately and triage incidents to ensure all parties are notified as required.
- Maintain cleanliness of the facility while ensuring compliance with health, safety, and fire codes. Identify issues requiring maintenance and work orders and forward them to appropriate personnel. Monitor for contraband.
- Work effectively with coworkers as a team.
- Utilize the onsite supervisor or the on-call system for guidance and support from supervising staff when any unusual, serious, or critical incident occurs.
- Respond to requests from clinical/counseling staff.
- Attend work as scheduled.
Requirements:
- Preferred but not required: High School Diploma or GED.
- Minimum of 1 year experience in one or more of the following:
- Residential or Outpatient Treatment
- Working with adults with mental illness/substance use disorders.
- Social Services
