Job Summary:

We're seeking a skilled and compassionate RN Case Manager to join our team. In this role, you'll work closely with physicians, patients, and families to coordinate care plans, manage complex disease states, and provide education and support to promote healthy outcomes.

Key Responsibilities:

  • Coordinate care plans with physicians, patients, and families to ensure seamless transitions and optimal health outcomes.
  • Manage complex disease states, including chronic conditions and comorbidities, to prevent hospitalizations and improve quality of life.
  • Provide education and support to patients and families on disease management, medication adherence, and healthy lifestyle choices.
  • Collaborate with multidisciplinary teams to develop and implement care plans that address the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate information exchange.

Requirements:

  • Bachelor's Degree in Nursing (BSN) and current RN licensure in Connecticut.
  • 2+ years of experience in case management, with a focus on complex disease states and telehealth.
  • Strong knowledge of NCQA/ACO/CDO regulations and standards.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to travel to Farmington, CT for training and meetings as needed.

Preferred Qualifications:

  • Master's Degree in Nursing (MSN) or related field.
  • Certified Case Manager Certification (CCMC) or related certification.
  • 5+ years of experience in case management, with a focus on complex disease states and telehealth.
  • Bilingual or multilingual skills, with the ability to communicate effectively with patients and families from diverse backgrounds.
