Job Summary

We are seeking a skilled Community Health and Wellbeing Nurse Specialist to join our team at Mount Carmel, a member of Trinity Health. This is a rewarding opportunity for a dedicated professional to provide comprehensive clinical nursing support to mothers and babies after discharge.

About the Role

This part-time position involves collecting and documenting vitals, medical history, allergies, and current medications in homes, as well as providing resources, referrals, and education tailored to each patient's unique needs. The ideal candidate will have a strong ability to work independently and efficiently in various settings, including homes and community locations.

Responsibilities
  • Provide post-partum assessment for mom and infant, including breastfeeding assistance and depression screening
  • Offer emotional support and follow-up with patients in need of short-term case management
  • Collaborate with local community resource agencies to ensure patients receive necessary services
Requirements

To be considered for this role, applicants must possess:

  • A graduate degree from a school of nursing, with a BSN preferred
  • A valid license to practice as a Registered Nurse in the State of Ohio
  • A minimum of 3 years' experience in a related field
