We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at EvergreenHealth. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities- Provide direct patient care, assessing the comprehensive nursing needs of assigned patients and coordinating their education process.
- Initiate patient start of care through established admission processes and continuously plan, implement, and evaluate patient care.
- Participate in the development and implementation of patient and family/significant other plans of care, working collaboratively with physician partners and other interdisciplinary team members.
- Effectively communicate patient information, safety issues, and quality concerns to immediate supervisors and team members.
- Deliver nursing care within established standards of care, hospital, and unit guidelines, policies, and procedures.
- Bachelor's in Nursing (BSN) required within 5 years of employment if applicable.
- Licensed as a Registered Nurse in the State of Washington.
- Current Healthcare Provider BLS certification by date of hire.
- Current Washington State Driver's license and proof of insurability.
