Job Summary:

As a Home Health Nurse Specialist at EvergreenHealth, you will provide comprehensive nursing care to patients in their homes, promoting patient-centered care and collaboration with interdisciplinary teams. This role requires a strong commitment to delivering high-quality care and a passion for working in a dynamic, patient-focused environment.

Key Responsibilities:
  • Assess and develop individualized care plans for patients, incorporating their unique needs and goals.
  • Provide direct patient care, including wound care, medication management, and education on self-care and disease management.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and optimal patient outcomes.
  • Communicate effectively with patients, families, and caregivers, ensuring clear understanding of care plans and promoting patient engagement.
  • Participate in ongoing education and professional development, staying current with best practices and evidence-based guidelines in home health care.
Requirements:
  • Bachelor's degree in Nursing (BSN) required within 5 years of employment.
  • Current RN licensure in the State of Washington.
  • Current Healthcare Provider BLS certification.
  • Valid Washington State driver's license and proof of insurability.
