Clinical Pharmacist PRN

We are seeking a highly skilled Clinical Pharmacist PRN to join our team at Methodist Hospital Stone Oak. As a Clinical Pharmacist PRN, you will play a vital role in providing pharmaceutical care to patients of all ages and population groups.
Responsibilities- Consult with nursing and medical staff on medication orders to ensure appropriate prescribing, route, method of administration, and length of therapy.
- Monitor patients for age-appropriate response to drug therapy and identify any adverse events from drug therapy.
- Perform computer order entry of medication orders and maintain patient medication profiles.
- Conduct therapy monitoring and other activities associated with the provision of pharmaceutical care, including formulary auto-substitution.
- Provide guidance to technical staff on performance of pharmacy services and supervise technical support personnel in drug preparation and distribution systems.
- Perform accurate final dispensing checks of all medication orders and complete required documentation.
- Comply with applicable federal and state law, pharmacy standards of practice, and US Pharmacopeia guidelines for compounding of sterile preparations.
- Participate in activities targeted by the department, including Clinical Pharmacy Reports, Medication Utilization Evaluation (MUE), food/drug interaction monitoring, patient counseling, etc.
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree.
- Minimum 1 year of pharmacy hospital experience preferred.
- Current Registered Pharmacist licensed by the Texas State Board of Pharmacy or licensed pharmacist in another state with proof of Authorization to Test (ATT) from the Texas State Board of Pharmacy.
