At Astyra Corporation, we are seeking a skilled Registered Nurse to join our team as a Home Health Nurse Assessor. This is an exciting opportunity to work in a dynamic and innovative healthcare organization, providing essential services to North Carolinians.
About the RoleWe are looking for a highly motivated and experienced RN to conduct assessments and provide guidance on personal care services (PCS) to NC Medicaid beneficiaries. The successful candidate will be responsible for determining eligibility for Medicaid-funded PCS, ensuring that services are provided on a needs basis, and maintaining high standards of privacy and dignity.
Key Responsibilities- Conduct assessments to determine beneficiary eligibility for PCS using state-approved standardized assessment tools.
- Ensure that PCS are provided on a needs basis, taking into account the severity of medical conditions, functional disabilities, physical or cognitive impairments.
- Maintain high standards of privacy and dignity during assessments.
- Complete assessments within established timeframes, including new, expedited, annual, change of status, mediation/appeals, reconsideration review, and derivative assessments.
The ideal candidate will have a strong background in home care settings, with experience conducting PCS assessments and working with community-based individuals needing personal assistance with ADL and IADL tasks. Proficiency in Microsoft Excel, Word, and Outlook, as well as the ability to utilize computer equipment and web-based software, is also essential.
Requirements- Registered Nurse, licensed by the state of North Carolina.
- Minimum of two years' experience in home care setting.
- Computer proficiency in Microsoft Excel, Word, and Outlook.
- Experience with community-based individuals needing personal assistance with ADL and IADL tasks.
- Experience conducting PCS assessments.
