MDS Coordinator

We are seeking a highly skilled and detail-oriented MDS Coordinator to join our team at Macon Rehabilitation and Healthcare Center. As a key member of our clinical department, you will play a critical role in ensuring the timely and accurate completion of the RAI and Care Management process.
Key Responsibilities- Collaborate with the Interdisciplinary Team to assess resident needs and develop effective care plans.
- Work closely with Regional Clinical Reimbursement Consultants to ensure compliance with State and Federal regulations.
- Coordinate daily care management meetings to review resident care and set Assessment Reference dates.
- Enforce accurate and timely resident assessments, designating responsibility for completing sections to the Interdisciplinary Team.
- Complete accurate coding of the Minimum Data Set (MDS) with information obtained through medical record review, observation, and interviews with facility staff, residents, and family members.
- Maintain tracking systems for MDS schedules, including time frames and due dates.
- Enter resident information into computerized MDS programs in a timely and accurate manner.
- Coordinate care plan conferences with the Interdisciplinary Team, residents, and families.
- Obtain, review, and maintain State and Federal reports, making timely corrections as needed.
- Coordinate education related to the RAI/Care Management process for facility staff.
- Submit required documentation to the State database and other entities per company policy and State and Federal regulations.
- Complete required forms and documents in accordance with Company policy and State and/or Federal regulations.
- Attend Interdisciplinary Team meetings and other meetings to gather information, communicate changes, and maintain and update records.
- Ensure Medicare compliance by contacting the Regional Clinical Reimbursement Center (RCRC) as needed.
- Stay up-to-date with knowledge related to data entry and computer technology.
- Participate in quality assurance activities.
- RAC Certified
- RN required
- Must be in good standing with the Board of Nursing
- Knowledge of Medicare process preferred
- Current State nurse license
- CEU's as required by the State
